Your first step is to call our office and set up a consultation. Consultations are a great opportunity for you to meet your dentist and get to know him or her better. Your dentist can talk with you about the different procedures that are offered and how you can benefit from them.
New Patient Paperwork and Documentation
Since it will be your first time at My Dentist, please complete the New Patient form. Evaluating your medical history allows your dentist to get a better understanding of your dental care needs. The form will take some time to fill out, so we recommend coming a few minutes early to your appointment. If you prefer to fill these forms out before you arrive, please click on the following link:
New Patient Form
All first-time patients will undergo a comprehensive exam. Your dentist will listen to your chief complaints, concerns and expectations. The dentist or hygienist will clean your teeth and check for cavities and gum disease. He or she will also evaluate your risk of developing other oral health problems, as well as check your face, neck and mouth for abnormalities. A dental exam will also likely include dental X-rays.
X-rays are a means of diagnosing problems that can otherwise go unnoticed. Tooth decay, abscesses, impacted teeth, or jawbone damage will all be visible on an x-ray.
After your dental exam, the dentist or hygienist will discuss your oral health, including your risk of tooth decay, gum disease and any other oral health problems, and preventive measures you can take to improve and protect your oral health.
